Referrals & self-referral

To start to access our services, you can either self-refer or be referred by your local GP, Social Worker or anyone in your Care Team.

To self-refer

It couldn't be easier to join one of our Reachout, Reachout Youth or Hub services.

To self-refer, please take the following steps:

1. Visit 'Our Services' pages to see what project suits you the most.
2. To join Reachout or the Hub, download one of our self-referral forms (below) and bring the form with you to one of our sessions. 
3. Contact us if you have any questions.
4. To join Reachout Youth, simply complete our confidential enquiry form to get a call back.
